Note: This book contains graphic images of severe cases of lymphedema. Book Review The 4th Edition of this pocket sized soft cover book is a further improvement on the previous 3 excellent editions. The text is concise, to the point, well sequenced and presented such that almost any question you are seeking an answer for from the anatomy, physiology, pathology, aetiology, assessment, treatment or management of lymphoedemas and associated syndromes can be easily found. What particularly makes this new edition more user friendly than the prior ones is the addition of a large number of full color examples of the conditions and situations the treating physician or lymphoedema therapist will come across. The color pictures are well chosen and representative and certainly well supported by high quality descriptive text. This addition makes truth of the statement “a picture is worth a thousand words”, but only when properly chosen as the editors have done. In a book of just over 500 pages it’s often not possible to cover a large range of topic areas in depth but some-how this one seems to be able to do it! You still will need to consult further specific texts if you really want to delve into the minutiae of any issue but at the end of each chapter there is an excellent list of references updated to 2007 so you can easily find and follow your wanderlust! There are obviously some personal biases and preferences with respect to what is presented by the editing authors, and certainly this is expected (and necessary since the experts views must be heard and valued) but what is great to see is the acknowledgement and inclusion of other views and directions, and as I mentioned earlier, you can follow your needs through the excellent references. Lymphoedema rarely occurs in isolation. It often is in concert with other disorders or is underlain or overlies them. The areas of lipoedema, phleboedema/lymphoedema, myxoedema, and the large range of angiodysplasias are also well covered and what is excellent in this edition is their clear linkage to lymphoedema. These inclusions make it very much easier for the practitioner to undertake a differential diagnosis, then confirm its correctness often through comparative tables and importantly with the aid of the larger range of clear pictures. Of course there is no benefit to a differential diagnosis if you do not know how to undertake a good examination of your patient and this is where I believe this text excels. The examination methods chapter is very comprehensive ranging from what a therapist can do with their own resources such as palpation, volumetry and tonometry through to the more expensive and potentially more accurate methods of lymphoscintigraphy, computed tomography and MRI. The pros and cons of these are clearly indicated, but it does show that no matter what you have or can do, the information is valuable in gaining a clear picture of the presenting patient’s condition. There is an excellent chapter on bio-electrical impedance spectroscopy (which is an increasingly well evidenced means of early and accurate diagnosis of tissue fluid levels) but it’s a pity that it seems a bit out of context at the end of the book. As an Editor of a number of International Journals one always looks for things to criticize but in this case it’s very difficult. The text is clearly written, arguments and existing knowledge are well presented and sequenced, the context of our current knowledge is shown, our current knowledge is indicated and what we can look forward to is presented. The index is perhaps a little brief but at least you are lead to the key areas of the chapters. Full physical addresses of the authors are given at the end of the book so you can communicate with them if needed. It’s a book which I’d be happy to have in my pocket for quick and clear reference. Its one which can confirm your thoughts for the common situations but which can provide the necessary details for those difficult and complex cases and show you what you need to do and what can be done for them!
